fix(anthropic): Token reporting

[alexander-alderman-webb]

Description

Stop double accumulating tokens, as Anthropic returns cumulative tokens in it's streaming chunks.

Adapts logic from the Anthropic Python SDK, and introduces a _RecordedUsage class to mirror Anthropic's SDK more closely, as the SDK mutates an instance as well.
